Responsibilities:
- Client Care: Conduct initial assessment and establish Plan of Care.
- Communication: Educate families on how to care for their loved ones.
- Evaluation and Documentation: Conduct reassessment of clients on a regular basis.
- Supervise: home care staff and maintain documentation.
Requirements:
- Current RN license.
- Professional, reliable, and personable.
- Strong communication and organizational skills.
- Able to work independently and collaboratively.
- Experience working with the elderly population.
